The date is November 24th, 2008. You have just started a job as an analyst working on the President elect's health care transition team. The director of the transition team has asked you to draft a memo to flesh out the health priorities for the new Administration on a major health policy issue. Select a candidate and an issue area (from the list below) and identify the major policies or strategies that the next Administration could develop to advance this issue. Make sure to include evidence and analysis to support your recommendations. Your priorities and strategies should be consistent with the proposals forwarded by the candidates in the campaign. You should also address the challenges in implementing your recommendations, such as budgetary and political considerations, delivery system issues, and how different stakeholders and constituencies would perceive the proposals.

Description:
The APTR-CDC Post Doctoral Fellowship in STD Prevention emphasizes epidemiology, behavioral science, health services research, and prevention of STDs to train today’s researchers to become tomorrow’s public health leaders at the local, state and national levels. Fellows gain leadership experience in public health practice and policy, have access to state-of-the-art technology and national databases, and will train with leading experts in the field of preventive medicine and public health.

Background/History:
The Division of STD Prevention (DSTD), at the Centers for Disease Control and Prevention, provides national leadership through research, policy development, and support of effective services to prevent sexually transmitted diseases (including HIV infection) and the related complications such as enhanced HIV transmission, infertility, adverse pregnancy outcomes, and reproductive tract cancer. DSTD assists health departments, health-care providers and non-governmental organizations, and collaborates with other governmental entities through the development, syntheses, translation, and dissemination of timely, science-based information; the development of national goals and science-based policy; and the development and support of science-based programs that meet the needs of communities. This mission is accomplished through the provision of technical and financial support to 65 STD prevention programs nationwide; including the 50 states, 7 large cities, and 8 territories. STD prevention activities supported include disease surveillance, disease intervention (counseling and partner notification services), school and community based educations programs, and program relevant behavioral, epidemiologic, clinical, health services, and evaluation research. At the national level, DSTD
supports technical assistance, laboratory, clinical, and program training, operational programs, policy coordination, guidelines development and prevention research.

Fellowship Assignment Description:
Several infectious disease and other sub-specialty postdoctoral fellowship programs emphasize STDs in trainee research experience and career development. However, the majority have emphasized microbiology, immunopathology, diagnosis and clinical management more than epidemiology, prevention, and service delivery. While existing programs have been successful in creating a national pool of superior academic investigators, trainees usually remain in an STD research setting or move out of the field altogether and have not assumed positions of leadership and responsibility in state or local health departments. This STD prevention fellowship emphasizes epidemiology, behavioral science, health services research, and prevention of STDs.
Under supervision and mentorship, fellows will pursue one or more research projects directly relevant to careers in public health, STD epidemiology, and prevention. Examples include research in provision of STD prevention services, research in STD/HIV interaction, analysis of traditional or innovative approaches to STD prevention and control such as partner notification or peer counseling; and behavioral intervention research.

Required and/or Desired Skills and Knowledge:
This fellowship is a post-doctoral level experience. Preferred medical specialties include infectious diseases, OB/GYN, family medicine, adolescent medicine, preventive medicine or internal medicine. While primarily designed for individuals with medical training, non-medical post-doctoral candidates, including individuals trained in public health, epidemiology, or one of the behavioral sciences, health services research or nursing are also eligible and may be considered.

Applicants must be postdoctoral with an M.D., D.O., Ph.D., Dr. P.H. or equivalent degree. A curriculum vitae and three letters of recommendation are required. Strong preference will be shown to applicants currently involved with a university-affiliated research component. Applicants involved in university-affiliated research should demonstrate an emphasis in one or more of the following as related to STD: epidemiology, behavioral science, prevention, and/or service delivery.

Applicants should provide a personal essay outlining career goals with particular emphasis on future involvement in STD prevention.

Expected Benefits:
The primary goal of this fellowship is to provide a training experience for individuals who, upon completion, can be appointed to universities, state or local health department STD prevention programs. This is critical to developing and reshaping a balanced STD prevention focus. The individual fellow will be assigned specific program-relevant research during the fellowship. This unique experience will help to develop a cadre of experts in STD prevention with an understanding of the public health system in this nation, and who will be able to address STD prevention needs across public and private sectors.

Period of Assignment:
The term of each Fellowship is one year, with the possibility of a one year renewal, for a total of two years maximum.

About the Employer:
The APTR Preventive Medicine and Public Health Fellowship Program is a collaborative initiative between the Association for Prevention Teaching and Research (APTR) and the Centers for Disease Control and Prevention (CDC). The primary objective of the program is to provide leadership training in public health practice and policy to preventive medicine and primary care residents; master’s, doctoral and post-doctoral level graduate students; and early career professionals (with a minimum of a master’s degree). Fellowship opportunities are located at the Centers/Institutes/Offices (CIOs) of the CDC.

Additional studies are designed to identify factors that influence cancer patients’ survival and their quality of life. Themes to research conducted within the Program in Epidemiology are the use of population-based study designs and the involvement of multi-disciplinary research teams.

The Program invites applications for two postdoctoral fellowship positions to work with faculty at the FHCRC and University of Washington in the design, conduct, and analysis of cancer epidemiology studies. Fellowships can be renewed annually for up to three years’ duration.

Qualifications

The Departments of Epidemiology and Nutrition at the Harvard School of Public Health (HSPH) have a long tradition of excellence in epidemiologic research relating to cardiovascular health.

An outstanding candidate is sought to work on original research projects supported by the National Institutes of Health on the effects of dietary habits on cardiovascular risk factors and disease outcomes. The research program and training will be directed by Dariush Mozaffarian, a cardiologist at Brigham and Women’s Hospital and Assistant Professor of Medicine and Epidemiology at Harvard Medical School and Harvard School of Public Health. Dietary factors of particular interest include fish, omega-3 fatty acids, trans fatty acids, mercury, and selenium, and outcomes of interest include intermediary endpoints such as hemodynamics, glucose-insulin metabolism, endothelial function, adiposity, and inflammation, and clinical endpoints such as coronary heart disease, sudden death, stroke, heart failure, atrial fibrillation, and diabetes. Gene-diet interactions can also be investigated. Major emphasis will be placed on leading original analyses and manuscripts; career development; and transition toward independent funding and an independent research career. Eligibility
Doctorate (PhD/DrPH/ScD) in Epidemiology or Nutrition, and/or MD (preferably with Medicine/Cardiology training) with MS/MPH in Epidemiology. Fellows are typically accepted for two years (conditional on performance); a third year may be considered.

Postdoctoral Position in Spatial Epidemiology

We are inviting applications for a postdoctoral researcher with expertise in spatial epidemiology to work on a project examining the influences of physical and social landscapes on obesity in rural communities. This research is supported by the USDA National Research Initiative Competitive Grants Program, and the position is funded for three years. The position will be located in the Geographic Information Science Center of Excellence at South Dakota State University, and will also involve collaboration with the E.A. Martin Program in Human
Nutrition. The postdoctoral researcher will be responsible for developing a national database of rural environments, analyzing environmental influences on obesity, and disseminating results via conference presentations and published articles.

Required Qualifications: A Ph.D. in epidemiology, geography, biostatistics, or a related field is required. Candidates must be able to take a lead role in conducting statistical analyses of geospatial datasets. Experience using GIS software to manage and process large geographic databases is also essential.

Desired Qualifications: Preference will be given to applicants who have experience in the application of spatial statistics to analyze public health data, knowledge of multivariate statistical techniques, and a strong interest in applying geospatial analysis methods to study interactions between humans and their environment.

The best research topics in statistical methods for design and analysis of epidemiologic studies are motivated by substantive studies. I am looking for a postdoc who is interested in participating in substantive research in epidemiology, including genetic and molecular epidemiology, and has the background to identify and investigate the key methodologic problem faced in these research areas.

The Division of Cancer Epidemiology and Genetics offers a wide range of opportunities for involvement in projects. For example, I play a key role on a number of exciting substantive projects, including several genome-wide association studies, cohort and case-control studies that address questions related to prevention and etiology of disease and to clinical practice, and a randomized trial of a vaccine against human papillomavirus. Each project raises methodologic questions that are challenging and important beyond the specific application. Solving these problems in a way that will improve the science or lead to improved public health can be fun, worthwhile, and professionally rewarding.

In addition to strong quantitative skills, this position requires either solid statistical training or knowledge of methods for epidemiologic studies or genetic association studies.

This position is subject to a background investigation.

Or Genomics for Physician Investigators. An entry-level faculty training position in genetics or genomics is available beginning July 1, 2008. The position is funded through an institutional K-12 grant from the NIH/NHLBI and aims to develop research independence in the analysis of genome-wide genetic and gene expression data. Research projects will focus on the identification of genetic determinants of asthma/COPD for physician investigators. The successful applicant should have either outstanding quantitative skills, formal training in genetics or genomics, or both. He/she will be involved in all aspects of learning how to perform this type of research. Applicants should have experience in genetic epidemiology, genomics, and/or computer programming, and have an MD degree or the equivalent. Our laboratory uses family-based and case-control genetic study designs combined with high-throughput genotyping and gene expression technologies to elucidate the determinants of chronic airway diseases. We have established 10 well-characterized cohorts (totaling more than 10,000 subjects with DNA samples) for the study of asthma and chronic obstructive pulmonary disease. Our laboratory is equipped with high- and intermediate-throughput genotyping and gene expression platforms, including the Illumina Bead Station and ABI 7900 platforms, an ABI 3730XL DNA sequencer, robotics, an integrated LIMS system, and a 20-node SUN-based cluster. Our faculty has strong expertise in conducting large epidemiologic, genetic, and genomic studies, and extensive experience with analyses of genomic data. This is not a J-1 opportunity.
